Trauma. A bump, fall, sports injury, or other blow to the mouth can damage the pulp inside a tooth. Sometimes the injury seems minor at the time, and the tooth may not change color until weeks or months later. As the internal tissue (pulp) breaks down, blood byproducts can seep into the dentin, gradually darkening the tooth from the inside out.
If your child's tooth became discolored after a fall or other injury, it's a good idea to have it evaluated. Even injuries that happened some time ago can affect a baby tooth or the developing permanent tooth underneath it.

Tooth Decay. Untreated cavities can progress deep enough into a tooth to reach and infect the pulp, leading to darkening. In addition to discoloration, you may notice a visible cavity, sensitivity, pain, bad breath, or swelling.
Deep decay in a baby tooth may require pediatric pulp therapy or a baby tooth root canal to remove infected or damaged pulp and preserve the tooth when appropriate.
Affected or Nonvital Pulp. When the pulp inside a tooth becomes severely damaged or dies, the tooth may gradually change color, as a visible external signal that something is wrong internally — even when a child has little or no pain.
Importantly, a dark tooth does not always mean that the pulp is dead, and color alone cannot determine the health of the tooth. A pediatric dentist needs to examine the tooth and, in some cases, take an X-ray to understand what's happening beneath the surface.
A darkening tooth deserves a relatively prompt dental evaluation, particularly if the discoloration followed an injury or occurs along with pain, swelling, drainage, or other changes, since it can indicate pulp damage or infection that may lead to an abscess or, in some cases, spread to surrounding tissues if left untreated — even in a baby tooth that will eventually fall out on its own.
Can SDF Make a Child's Tooth Turn Black?
Yes—and this is an important exception for parents to know about.
If your child has received silver diamine fluoride (SDF) treatment for a cavity, the decayed portion of the treated tooth can turn dark brown or black. This discoloration is an expected result of SDF treatment and does not necessarily mean that the tooth's pulp is damaged.
SDF works by helping arrest active tooth decay, and the decayed area can become permanently dark after treatment. SDF primarily causes dark staining of the decayed tooth surfaces. Healthy tooth structure is generally much less likely to show this type of staining.
This means that darkening after SDF treatment is an expected treatment effect, whereas unexplained darkening or darkening after trauma should be evaluated by a pediatric dentist.

Why Do My Kid's Teeth Look Yellow?

Yellow teeth can have many causes, and some are less concerning than others. There are several possible reasons your child's teeth may look yellow:
Thinner enamel on baby teeth. Primary (baby) teeth naturally have thinner, more translucent enamel than permanent teeth, allowing more of the yellowish dentin layer underneath to show through. As a result, some baby teeth may naturally appear more yellow or creamy than parents expect. This can be a normal variation, although a dentist can help determine whether yellowing is related to normal tooth structure, plaque, staining, decay, or another cause.
Diet and surface staining. Pigmented foods and drinks, such as tomato-based sauces and certain juices, can sometimes contribute to surface discoloration and staining, particularly when combined with less-than-thorough brushing.
Frequent exposure to sugary foods and drinks can increase the risk of tooth decay, while frequent exposure to acidic foods and drinks can contribute to enamel erosion. Encouraging water between meals, limiting sugary drinks and snacks, and maintaining consistent brushing can help protect your child's developing teeth.

Certain medications. Some medications can cause tooth discoloration while teeth are developing. Other Tooth Discoloration Patterns Worth Knowing

White Spots

How Do We Evaluate Tooth Discoloration?

Sometimes the cause can be identified through a visual examination alone. In other situations, dental X-rays may be needed to evaluate the tooth's internal structures, assess the health of the pulp inside a darkened tooth, look for decay, or evaluate the developing permanent teeth.
